Established Patient Documents
If you are already a patient at NMC Health, you’ll need to fill out paperwork once a year to ensure our records are up-to-date with changes in your life. If it’s been one year or more since your last visit with us please print, fill out and bring the following documents with you along with copies of your ID and insurance cards when you come in for your appointment:
New Patient Documents
Welcome to NMC Health! As a new patient, there are some documents we’ll need you to complete before you can be seen by your doctor. Please print, fill out and bring these completed documents with you to your visit with copies of your ID and insurance cards:
Sports Physicals
Before you can play sports in school or for a league team, you have to have a sports physical.
NMC Health Immediate Care in Newton offers $20 physicals for student athletes and an appointment is not necessary.
Our family medicine clinics also offer sports physicals by appointment.
Payment is due at time of service. Your insurance will not be billed.
Don’t forget to bring this Sports Physical form or the one provided by your child’s school.
