We are currently seeking a Diabetes Clinical Education Coordinator for our Diabetic Education Department. In this role, you will oversee a program of education for clients of all ages with chronic disease, including but not limited to the diabetes continuum, cardiovascular and renal dysfunction, and weight problems according to the individualized needs of the client. This process involves a health and education assessment, development, implementation and evaluation of an education plan, considering psychosocial, economic, and language barriers. Duties are performed in a classroom or individual outpatient settings in collaboration with a professional team. This position is responsible for all aspects of the accredited Diabetes Self-Management Education and Training (DSME/T) program throughout the enterprise. The Educator incorporates professional development and community outreach as an integral part of professional practice.

This position reports to the Director of Outpatient Services.

This position is hourly (non-exempt).

 

QUALIFICATIONS

Education and Experience:

  • Minimum of a bachelor’s degree in nursing or dietetics. Master’s preferred.
  • Maintains current knowledge of diabetes education and/or certification in area of specialty.
  • Minimum 5 years clinical experience including experience in treatment and education of diabetes within the last 2 years.
  • Formal coursework and/or experience preparation in program management.
  • Formal coursework or credentials in principles of patient-centered learning across the age spectrum preferred.
  • Proven ability to lead and supervisor other professionals.
  • Spanish proficiency desirable; will accept documented experience with diverse ethnic groups.

 

Knowledge and Skills:

  • Knowledge and experience with current pediatric and adult learning theory in relation to chronic disease self-management, especially as it relates to the spectrum of diabetes.
  • Skill with facilitating group and individual interactions to maximize the education experience.
  • Demonstrated problem-solving skills with the ability to prioritize work and manage time effectively.
  • Effective verbal and written communication skills with health care team and diverse client populations.
  • Special equipment utilized: glucose monitors, insulin pumps, and diabetes-related products.
  • Skilled using an electronic medical record, MS Word, Excel, Outlook, and the Internet.

Licensure, registration and certification:

  • Current credentials as a Registered Nurse or Registered, Licensed Dietician required.
  • Maintains current credentials as a Certified Diabetes Educator
